Required Document Self Attested

Candidates shall submit their applications along with the handover of the following documents to the DM&HO Office, Rajamahendravaram duly making self-attestation on the Xerox copy of the Certificate.

• Copy of SSC Certificate (Proof of Date of Birth)
• Should enclose a copy of the resolution & approval by the VHSNC of the respective area.
• Copy of the Latest Caste Certificate issued by Mandal Revenue Officers concerned in the case of SC, ST, and BC, otherwise they will be treated as OC.
• Copy of the certificate of Married / Widow / Divorced / Separated for consideration priority.
• Residence certificate / Aaadhar card/ration card etc.,
